Sunshine Windows Manufacturing, Inc. ACCEPTANCE No.: 08-0512.07 APPROVED NOV 1 5 2000 EXPIRES October 15, 2005 NOTICE OF ACCEPTANCE SPECIFIC CONDITIONS 1. SCOPE 1.1 This Notice and means the Notice of Acceptance No. 96-1008.03, which was issued on October 14, 1997, approves an aluminum fixed window, as described in Section 2 of this Notice of Acceptance, designed to comply with the South Florida Building Code, Third Edition, Miami-Dade County, Florida, and the applicable requirements of the 1998 Florida Building Code, as listed by SBCC Chapter 12, and the Design Pressures listed in the approved drawings. 2. PRODUCT DESCRIPTION 2.1 The Series 4250 Circle Top Aluminum Fixed Window and its components shall be constructed in strict compliance with the following: a) 90-1008.03, Sheet 1 Unit 1 of 3, signed and sealed by Michael J. Moran, P.E., bearing the Miami-Dade County Product Control Approval stamp, with the Notice of Acceptance number 96-1008.03, and dated October 14, 1997, issued to Sunshine Windows Manufacturing, Inc., in order to offer it to the approved drawings. 3. LIMITATIONS 3.1 This approval applies to single unit applications only, as shown in approved drawings. 3.2 For Design Pressures Refer to Sheet 2 Unit 2 of 3, signed and sealed by Michael J. Moran, P.E., bearing the Miami-Dade County Product Control Approval stamp, with the Notice of Acceptance number 96-1008.03, and dated October 14, 1997, issued to Sunshine Windows Manufacturing, Inc., in order to offer it to the approved drawings. 3.3 Circle Top or Eyebrow Top Shapes. 4. INSTALLATION 4.1 The aluminum fixed window with mullins and its components shall be installed in strict compliance with the approved drawings. 4.2 Wind driven rain protection system (shutters): the installation of this unit will requires a hurricane protection system. 5. LABELING 5.1 Each window unit shall bear a permanent label with the manufacturer's name or logo, city, state and following statement: "Miami-Dade County Product Control Approval". 6. BUILDING PERMIT REQUIRED 6.1 This Notice of Acceptance shall be accompanied by copies of the following: 6.1.1 The Notice of Acceptance. 6.1.2 Duplicate copies of the approved drawings, as identified in Section 2 of this Notice of Acceptance, and the design pressers indicated for the proposed installation. 6.1.3 Any other documents required by the Building Official or the South Florida Building Code (SFBC) in order to properly evaluate the installation of this system.
