DIAL DOOR COMPANY, INC. ACCEPTANCE NO: B-042601 APPROVED: OCT 04 2004 EXPIRES: OCT 10 4, 2006 NOTICE OF ACCEPTANCE; SPECIFIC CONDITIONS 1. SCOPE 1.1 The DAD 9-Mile Sectional Door as described in Section 2 of this Notice of Acceptance, and as is de- to comply with the South Florida Building Code, 1994 Edition for Miami-Dade County (SRBC/DC) for the locations where the pressure requirements, as determined by SRBC Chapter 21, do not exceed the design pressure of the product as shown on the approved drawings. 2. 2.1 The DAD 9-Mile Sectional Door and its components shall be manufactured in strict compliance with the following documents: Drawing No. 01-03, titled "Sectional Garage Door," prepared by Miami-Dade County Product Control Division, and the "SRBC Approved Product List" prepared by Miami-Dade Product Control. Approval stamp with the Notice of Acceptance number and approval date by Miami-Dade Product Control Division. These documents shall hereinafter be referred to as the approved drawings. 3. 3.1 The manufacturer shall not alter or modify these drawings or the approved drawings shall qualify and / approved, as long as all components are exactly as described in this approval. 3.2 This approval requires the manufacturer to do nothing of all cells need to fabricate these panels under a / license agreement with the owner of the patent rights.
