P.O. Box 2000 Hallandale, FL 33002 SCOPE: This NOA is issued under the applicable rules and regulations governing the use of registration numbers by the Board of Rules and Appeals (BR&A). The NOA shall be valid until the expiration date stated below. The Miami-Dade County Product Control Division (the Miami-Dade County) under the Authority of the BR&A, reserves the right to revoke or suspend this NOA at any time. The Miami-Dade County Product Control Division reserves the right to revoke, modify, or suspend this NOA if it is determined by Miami-Dade County Product Control Division that this product or material fails to meet the requirements of the applicable building code. This product is approved as described herein, and has been designed to comply with the High Velocity Hurricane Zone of the following Florida Building Code, Section 150.403 (3): Miami-Dade County, Florida. APPROVAL DOCUMENT: Drawing No. 0899-4491, United States (US) - Miami-Dade County, Florida (MDC) 1st Re-Approval, dated March 26, 2014, issued to Sunshine Welding Manufacturing, Inc., 10000 NW 41st Street, Miami, FL 33166, a Division of Miami-Dade County Product Control Review, signed and sealed by Fernando Hernandez, P.E., Engineering Manager, Product Control Review, with the Notice of Acceptance number and expiration date on this Miami-Dade County Notice of Acceptance. VALIDITY OF NOA/RATING: None LABELING: Each unit shall bear a permanent label stating "Miami-Dade County Product Control Approved", unless otherwise noted herein. TERMINATION OF THIS NOA: This Notice of Acceptance will expire on March 26, 2020. The acceptance of this product, its rating, advertising or any other representation of this NOA as a Miami-Dade County Approved Product, shall be terminated immediately upon notification to the holder of this NOA, if there has been a revision or change in the product, its rating, advertising or any other representation of this NOA as a Miami-Dade County Approved Product, or if any conditions of this NOA are not complied with. The NOA number preceded by the words Miami-Dade County, FL, and followed by the expiration date may be displayed in advertising literature. If any portion of this NOA is displayed, then it shall be legible and readable. DISPOSITION: A copy of this notice NOA shall be provided to the user by the CO/AC.
