This NOA is issued by the Product Control Division (the "PCD") at the request of the Applicant for the product described herein and in compliance with the applicable building code in effect in Miami-Dade County. This NOA shall be valid for 5 years after the expiration date stated below. The Miami-Dade County Product Control Division (the "PCD") under the Act (as amended) and the Rules and Regulations of the Product Control Division (the "PCD") under the Act (as amended). This product or material fails to perform in the use for which it is intended, the manufacturer will bear the expense of such testing and the PCD reserves the right to revoke, modify, or suspend the use of such NOA. The holder of this NOA reserves the right to make changes in materials or design of the product or material so long as the requirements of the applicable building code. This product is approved as described herein, and has been designed to comply with the Florida Building Code, including the High Velocity Hurricane Zone. DESCRIPTION: Vented by Miami Spanish, Clay roof Tile LABELING: Each unit shall have a permanent label of the manufacturer's name or logo, city, state and including the High Velocity Hurricane Zone. REINSTALLMENT: This NOA shall be considered after a renewed application has been filed and there has been no change in the applicable building code negatively affecting the product. If there has been a revision or change in the applicable building code negatively affecting the product, a new application must be filed. TERMINATION: This NOA shall be valid until the expiration date. If there has been a revision or change in the applicable building code negatively affecting the product, this NOA shall be terminated. Failure to comply with the requirements of this product or process. Misuse of this NOA as an endorsement of NOA. REASSESSMENT: The NOA number preceded by the words Miami-Dade County, Florida, and followed by the expiration date may be displayed in advertising literature. If any portion of the NOA is copied or duplicated, it shall be with the written consent of the Product Control Division. TERMS OF USE: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors and shall be available for inspection at the job site at the request of the Building Official.
