The documentation submitted has been reviewed and is accepted by the Building Code Official. This NOA is valid in Miami-Dade County and other areas where allowed by the Authority Having Jurisdiction (AHJ). The NOA shall be valid after the expiration date only if: The Miami-Dade County Product Control Division (or, Miami-Dade County Product Control Division if the Miami-Dade County fails to perform in its duties) has not revoked the quality assurance program; The product has not been altered or changed in design, material, and the AHJ may deny final acceptance; The manufacturer has not changed the product or control, which denied the County Division that this product or works with compliance, if it is determined by Miami-Dade County. This product is engineered, tested, and has been assigned to comply with the High Velocity Hurricane Zone of the South Florida Building Code. DESCRIPTION: GAF Conventional Built-Up Roof System for Wood Deck. LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and following statement: "Miami-Dade County Product Control Approved", unless otherwise stated herein. RENEWAL: If the NOA shall be considered as a renewal application but has been and there has been no change in the applicable building code or regulations affecting the performance of this product. TERMINATION: This NOA will cease to exist after the expiration date or if there has been changes or changes in the product, its rating, advertisement or use of the product or process. Additionally, it terminates this NOA. Failure to comply with any section of this NOA shall be grounds for cancellation and removal of NOA. APPROVAL: This NOA is issued under the laws of the State of Florida. This NOA, issued and followed by the County Division, shall be valid for the use of the product. The NOA number provided by the words "Miami-Dade County" and shall be displayed in advertising literature. If any part of the NOA is displayed, then it shall be done so in entirety. INSPECTION: A copy of this revised NOA shall be provided to the user by the manufacturer of the distributor and shall be available for inspection at the job site at the request of the Building Official.
