It is issued by and only be used by the Miami-Dade County Product Control Division as directed by the Board of County Commissioners of the State of Florida, through the Miami-Dade County Product Control Division, and the Authority Having Jurisdiction (AHJ). This NOA shall not be valid after the expiration date stated below. The Miami-Dade County Product Control Division, the Miami-Dade County Product Approval Board, and the Miami-Dade County Building Code Appeals Board reserve the right to revoke this approval at any time. If this product is installed in a manner other than described in this NOA, the acceptance will be void. In the event of any conflict between this NOA and the applicable building code, the building code shall govern. DESCRIPTION: Single 5/8" OSB Sheathing, Decking Grade, Single Layer Wood Fiber Roof. APPROVAL DOCUMENT: Drawing No. 9941, title: "TBMG Series 500, Wood Deck" sheets 1 through 3 of 3, prepared by Mfr. dated 07/19/99, bearing the Miami-Dade County Product Control Approved stamp with the Miami-Dade County Product Control Division seal. MISSILE IMPACT RATING: None LABLING: Each unit shall bear a permanent label with the manufacturer's name, city, state and following statement: "Miami-Dade County Product Approved", unless otherwise noted herein. TERMINATION: This NOA will cease after the expiration date or if there has been a revision or change in the product's design, material or manufacture of the product or process. Misuse of this NOA as an endorsement of any product, trade name or method of construction, or to reproduce, verbatim or as reference, any part of this NOA, shall be cause for termination, and removal of NOA. ADVERTISMENT: The NOA number preceded by the words Miami-Dade County, Florida, and followed by the expiration date, shall be displayed in advertising literature. If any portion of the NOA is displayed, then it shall be done in its entirety. INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors at the time of delivery of the product to the job site.
